This Report and its contents are privileged, confidential and immune from discovery or admissibility at trial based upon RCW 70.41.200 et seq. and RCW 4.24.250. These Reports are exclusively used for peer review and quality improvement activities by the Hospital District to improve services and patient care. These activities include but are not limited to the ongoing evaluation of the quality of patient care provided by health care providers, evaluation of patient/visitor complaints, evaluation of the quality of services offered by hospital departments and personnel, and to evaluate the Hospital's compliance with applicable regulations, Joint Commission guidelines, the expected standard of care, and other appropriate safety and quality of care standards. Health care providers and individuals completing this Report are to candidly report events to facilitate ongoing quality improvement activities and the quality of care and services provided by the Hospital District. RCW 70.41.200 et seq. and RCW 4.24.250 make these reports privileged and confidential and state that they cannot be obtained during discovery in a legal action and are not admissible at trial in any legal action.

Confidentiality is maintained and protected; retaliation will not be tolerated. These reports offer opportunities for education, growth, and quality improvement throughout all hospital systems and services.
